Service Hour Form

Roughrider Scholarship Foundation Service Hour Projects require the participation/donation of time and hours that make a positive difference and an impact in the community. Items may not be purchased to meet the hour requirement. This form is used to document your participation in a community service activity. The signature of a sponsor or project supervisor is required.
